The SMI or Seriously Mentally Ill program offers a variety of
services within the organization.
SMI Center
The SMI Center delivers
services to adults age eighteen and older who are diagnosed with a serious
mental illness. Case Management services and the Therapeutic Day Program
are located at 477 E. Broadway. Case Management services are
available Monday through Friday from 8:00 AM to 5:00 PM.
Legend House
This is a 24-hour residential living setting with an eight-bed capacity.
This program is designed to provide a structured and transitional treatment setting and continuous
supervision for clients with a serious mental illness. Staff provide instruction in
the areas of rehabilitation and support services. All residents receive
case management services on-site.
Road House
This is a semi-supervised independent living apartment setting. Clients must have the ability to grocery shop and
cook their own meals in this setting. Medication monitoring is offered, if
needed.
Low-Income SMI housing
SMMHC, Inc. owns two 4-plex apartment complexes and to 3-bedroom
homes in the Apache Junction community. To be eligible for these housing
units, participants must meet HUD income guidelines, be able to independently perform all
activities of daily living and demonstrate the ability to take all medications
as prescribed independently.
SMI Adult Living Skills Programs (SASLS)
SMMHC operates two community-based, 3 bedroom homes in Apache
Junction for SMI adults who wish to live independently, but require intensive
supervision with their ADLs. Staff is available 24-hours on-site to assist
clients with their individual needs.
This program is also used for respite for SMI adults as
appropriate.
